We offer a Basic Plan and a Perio Plan to meet different dental care needs. Basic Dental Plan
Adults (18+)
$350/year
Children (3 -17y)
$250/year
Plan Covers:
One Full Set of X-rays (Once every 5 years).
One set of Bitewing X-rays (Once per year).
All Periodic, Comprehensive, and Limited Exams.
Two Prophylaxis Cleanings.
Two Fluoride Treatments.
Plus, a 20% discount on:
Sealants.
Fillings.
Crowns, Veneers, Bridges, and Dentures.
Root Canals
Extractions.
Exclusions and Limitations:
Plan Excludes Implants, Orthodontic Aligners, and Whitening Products. This plan may not be used in conjunction with any other insurance plan.
Any co-payment is required at the time of service.
Plan benefits must be used within 365 days after purchase.
Periodontal Dental Plan
Adults (18+)
$550/year
Plan Covers:
One Full Set of X-rays (Once every 5 years).
One set of Bitewing X-rays (Once per year).
All Periodic, Comprehensive, and Limited Exams.
Four Periodontal Cleanings.
One Full Mouth Debridement (as needed).
Two Fluoride Treatments
Plus, a 20% discount on:
Periodontal Therapy.
Sealants.
Fillings.
Crowns, Veneers, Bridges, and Dentures.
Root Canals
Extractions.
Exclusions and Limitations:
